Apointment & Testing

If you would like to be seen and evaluated for a hormone consultation, we recommend having testing done prior to your appointment. You may call the office (937-436-9821) and a prescription for blood work will be faxed to you. You may also pick up a copy at the office. Take the prescription to a local lab where your blood will be drawn. Make sure that the lab you choose is covered by your insurance. Some patient's insurance requires that their primary care doctor order the blood testing.

Patients who do not have insurance that covers testing, or who have a high deductible, may arrange to have their testing done through Dr. Glaser's office. Payment would be made to the Millennium Wellness Center at which time a special prescription would be given for blood work that allows CompuNet to bill the Millennium Wellness Center . The patient would then take this prescription to one of the CompuNet labs listed on this website. The cost of testing would be under $200.

Another option for patients without insurance or high deductibles would be to take the prescription for bloodwork to one of the LabCorp locations listed on this site. They would be eligible for special ‘patient pricing' arranged through Dr. Glaser and the Millennium Wellness Center . Patients must pay LabCorp at the time of services and make sure to notify the technician that they are eligible for ‘special pricing'. Complete lab testing should be under $250.

When the office receives the results of testing (10-14 days), the receptionist will call you and set up an appointment for a consultation. Make sure the office has your name and phone number! If you do not hear from the office within 14 days of testing, call and make them aware that you had testing done. You may also want to contact the lab and make sure the results of your testing were sent to the office.

Dr. Rebecca Glaser and the 'Millennium Wellness Center' are located at 7740 Washington Village Dr., Suite 110 of the Cornerstone Medical Building . This is located next to Champp's Restaurant, across from the Chop House.
